यस कम्पनीबाट जारी तपशिलमा उल्लेख भएको बिमालेखका बिमितले बिमालेख हराएको/नासिएकोले प्रतिलिपि बिमालेखको लागि निवेदन पेश गरेको हुँदा उक्त बिमालेख उपर कसैको हकदावी भए यो सूचना प्रकाशित मितिले १५ दिनभित्र कम्पनीको केन्द्रीय कार्यालयमा सबुद प्रमाणसहित हकदावी दर्ता गर्नुहुन सम्बन्धित सबैको जानकारीका लागि यो सूचना प्रकाशित गरिएको छ। यो अवधिभित्र दावी दर्ता नभएमा निवेदकको माग बमोजिम प्रतिलिपि बिमालेख जारी गरिने व्यहोरा समेत सम्बन्धित सबैका लागि जानकारी गराइन्छ।

Notice Regarding Claim
Since the insured of the insurance policy mentioned in the details issued by this company has applied for a duplicate insurance policy due to the loss/destruction of the original policy, this notice is published for the information of all concerned that if anyone has a claim on the said insurance policy, they should register their claim with valid proof at the company's central office within १५ days from the date of publication of this notice. It is also informed to all concerned that if no claim is registered within this period, a duplicate insurance policy will be issued as per the applicant's request.
