How to Extend Japan Medical Visa for Continued Treatment

The Japan Medical Visa Extension (在留期間更新許可申請) is an important process for Nepali citizens receiving ongoing medical treatment in Japan. This visa type, officially known as the Medical Stay Visa (医療滞在ビザ), is primarily issued for short-term (up to 90 days) or long-term (up to 1 year) medical purposes, including surgeries, chronic disease management, rehabilitation, and advanced checkups. However, if a patient’s recovery or treatment period extends beyond the visa’s validity, they can apply for a visa extension directly within Japan before the expiry date.

To qualify for an extension, the patient must have a genuine medical reason verified by a Japanese hospital or clinic. The hospital should issue a Medical Continuation Certificate explaining the patient’s health condition, ongoing treatment necessity, and estimated additional treatment duration. The visa holder must also demonstrate sufficient financial resources to support medical expenses, accommodation, and living costs during the extended stay, as Japan does not allow foreign patients under this visa to access public health insurance.

The required documents for extending a Japan Medical Visa include:

  • A filled Application for Extension of Period of Stay (在留期間更新許可申請書) form, available from the local immigration office or Japan Immigration Services Agency website.

  • Valid passport and residence card (if applicable)

  • Medical certificate or hospital-issued treatment continuation letter

  • Letter from the hospital specifying treatment necessity, estimated recovery time, and cost details

  • Proof of financial capability — recent bank statements, hospital deposit slip, or sponsor’s income certificate

  • Accommodation proof, such as hospital admission letter or rental document

  • Guarantor’s letter from the hospital or sponsor in Japan

  • A revenue stamp worth JPY 4,000 as the application fee

If the patient is accompanied by a family member or caretaker, their extension application should also include a relationship certificate (marriage or birth certificate) and an explanation of why their presence is medically necessary. The visa extension must be filed at least 2–3 weeks before the current visa expires to prevent overstaying, as overstaying in Japan may result in penalties, deportation, or denial of future visas.

The processing time for a medical visa extension is generally 2 to 4 weeks, depending on the immigration office and documentation completeness. Upon approval, the visa is typically extended for 3 months, 6 months, or up to 1 year, depending on the hospital’s recommendation and treatment plan. Patients are not allowed to engage in any kind of employment during this period, as the visa is strictly for medical purposes.

After successful treatment, the patient must return to Nepal. If additional follow-up treatments are needed, a new Japan Medical Visa can be applied for through the Embassy of Japan in Kathmandu, using updated hospital invitation letters and medical reports. For continuous medical visits or coordination with Japanese hospitals, professional assistance can ensure faster processing and error-free documentation.
